**Costco Optical is in-network with most plans and will be able to directly bill your insurance for glasses and/or contacts on those plans.
For eye exams, we are Open Access OUT-OF-NETWORK with vision insurance/discount plans. Open Access means you can still utilize your vision benefits at an out-of-network provider. We will print, fill out, and mail in the reimbursement forms to your insurance provider for you. You will receive a reimbursement check for your out-of-network benefits in the mail a few weeks later. We do all the work for you! Full exam fees due at time of service, many patients utilize their HSA/FSA accounts for this.
We have forms that we will submit for your OUT-OF-NETWORK reimbursement for the following insurance providers:

Aetna - No direct form. If you have a vision plan through Aetna, it is typically Eyemed or VSP which we do have a form for. Please confirm with Aetna which vision provider if you are unsure.
Ameritas - No direct form. If you have a vision plan through Ameritas, it is typically Eyemed or VSP which we do have a form for. Please confirm with Aetna which vision provider if you are unsure.

Humana - No direct form. If you have a vision plan through Humana, it is typically Eyemed which we do have a form for. Please confirm with Humana if you are unsure.
Lincoln - No direct form. If you have a vision plan through Lincoln, it is typically United Healthcare/Spectera which we do have a form for. Please confirm with Lincoln if you are unsure
Medical Mutual - We have a direct form for Medical Mutual “Traditional Vision Plan,” however they often have a separate vision provider, typically Eyemed or VSP which we do have forms for.
Medicare - No direct form. We are not in network with Traditional Medicare medical insurance. If your Advantage Plan goes through a vision plan listed here, we will have the forms.
Metlife - No direct form. If you have a vision plan through Metlife, it is often Superior, Eyemed or VSP which we do have a form for. Please confirm with Metlife which vision provider if you are unsure.

Summacare (Medicare) - We have the form for Summacare through Medicare. Often non-medicare summacare (no direct form) will go through a vision provider, typically Eyemed.

**Most vision provider plans have out-of-network benefits available, however it is up to your insurance company if you have these benefits and how much they are. We recommend calling them ahead of time if you want to confirm you have benefits and the amount they reimburse. After we submit for you, any reimbursement inquiries should be directed to your insurance company.
